Sclerosing Fibroadenoma With Atypical Ductal Hyperplasia Mimicking Invasive Carcinoma: A Case Report With Diagnostic Pitfall

Fibroadenoma (FA) of the breast is a benign fibroepithelial lesion rarely showing atypical epithelial overgrowth. We present case 50-year-old Japanese woman with sclerotic FA ductal hyperplasia (ADH)/ductal carcinoma in situ (DCIS). A small mass was detected during clinical examination upper lateral area left breast. Hematoxylin and eosin stain section needle core biopsy specimen showed trabecular growth cells without distinct myoepithelial lining stroma. Initial pathological diagnosis invasive no special type. The surgical specimens included well-bordered nodular similar histological findings to that specimen, but, highlighted by cytokeratin 5 (CK5) immunohistochemistry. tumor were diffusely ER-positive completely negative for CK5 immunohistochemical staining. Final based on results staining consultation between two pathology specialists as sclerosing ADH/DCIS. Awareness unique subtype important avoid misdiagnosis overtreatment.
